Among the several types of ocular inflammation associated to the rheumatic diseases, anterior uveitis is particularly common in the spondyloarthropathies, especially when associated to the presence of the HLA-B 27 genotype. We report the case of HLA-B 27 positive patient with undifferentiated arthritis, complicated with panuveitis and retinal vasculitis, that was refractory to the traditional imunossupressive treatment, and had a good clinical response with anti-TNF-alpha therapy.
HLA-B 27 subtypes among the Chukotka native groups. With respect to HLA-B 27 subtypes the indigenous populations from the eastern part of the Chukotka Peninsula are genetically more closely related to Caucasians than to Orientals.
Krylov, M. Revmatizma; Reveille, J. Health Science Center. What is new HLA-B 27 acute anterior uveitis? The disease is typically acute in onset, unilateral, nongranulomatous inflammation involving the iris and ciliary body, with a tendency to recurrent attacks. Environmental factors play a critical role in the pathogenesis of AAU; in particular, bacterial triggers have been strongly implicated in the development of this disease.
Topical corticosteroids and cycloplegic agents remain the cornerstones of treatment for AAU. Salazopirine and methotrexate are effective in decreasing recurrent attacks. HLA-B 27 can misfold to form cell-surface heavy chain homodimers B and induce pro-inflammatory responses that may lead to SpA pathogenesis.
We characterized a novel Bspecific monoclonal antibody to study its therapeutic use in HLA-B 27 associated disorders. The monoclonal HD5 antibody was selected from a phage library to target cell-surface B homodimers and characterized for affinity, specificity and ligand binding. Onset and progression of disease profiles were monitored during therapy. Cell-surface B and expansion of pro-inflammatory cells from blood, spleen and draining lymph nodes were assessed by flow cytometry.
Monoclonal antibodies targeting cell-surface B propose a new concept for the modulation of inflammatory responses in HLA-B 27 related disorders. We characterized a novel B—specific monoclonal antibody to study its therapeutic use in HLA-B 27 associated disorders. Methods The monoclonal HD5 antibody was selected from a phage library to target cell-surface B homodimers and characterized for affinity, specificity and ligand binding. Susceptibility and HLA-B 27 in post-dysenteric arthropathies.
A recent outbreak of bacillary dysentery in The Netherlands revealed that, despite the close association of HLA-B 27 with post-dysenteric or reactive arthritis ReA , not even in one family did all HLA-B 27 positive patients infected by an arthritogenic bacterium, develop ReA.
This dissociation shows. Full Text Available The HLA-B 27 gene is a major risk factor for clinical diseases including ankylosing spondylitis, acute anterior uveitis, reactive arthritis, and psoriatic arthritis, but its mechanism of risk enhancement is not completely understood.
The gut microbiome has recently been shown to influence several HLA-linked diseases. However, the role of HLA-B 27 in shaping the gut microbiome has not been previously investigated. In this study, we characterize the differences in the gut microbiota mediated by the presence of the HLA-B 27 gene. Further analysis of the data set revealed an increase in Prevotella spp. The finding that HLA-B 27 is associated with altered cecal microbiota has not been shown before and can potentially provide a better understanding of the clinical diseases associated with this gene.
Evaluation of hla-b 27 positive patients suspected of seronegative spondyloarthropathies. Among these patients, individuals found HLA-B 27 positive were investigated regarding clinical findings, age, and sex distribution.
In this study the frequency of HLA-B 27 antigen was The minimum age in males was 10 years and the maximum age in female was 70 years. Median age with seronegative SpAs findings Based on our results, the most frequent clinical manifestation, was peripheral joints arthritis There were no association between any of the major clinical manifestations and age or sex distribution.
These findings confirm the strong association of the HLA B 27 allele with various types of spondyloarthritis and suggests that HLA typing would help in the diagnosis of seronagative SpAs, specially ankylosing spondylitis with indeterminate clinical presentation and also in identifying at risk family members. The radiographic features of rheumatoid arthritis in HLA-B 27 -positive patients.
Rundback, J. Radiographs were reviewed in a group of nine patients with classical seropositive rheumatoid arthritis who on tissue typing were found to express the class I HLA-B 27 allele. Radiographs were analyzed with regard to whether or not they demonstrated radiographic features of 1 classical rheumatoid arthritis, 2 seronegative arthritis, or 3 mixed features of rheumatoid and seronegative arthritis.
Thus, the wide range of findings in our population indicates that the radiographic attributes are not specific enough to constitute a unique subpopulation of patients with rheumatoid arthritis. We stu The study was conducted in at the ophthalmology department of the University Medical Center of Groningen. Spondyloarthritis SpA does not display the typical features of auto-immune disease. Retrospective cohort study.
Mean age was Mean follow-up period was A reduction in the number of systemic medications was achieved in patients Infliximab, adalimumab, and etanercept were effective for treating and reducing the number of uveitis relapses in HLA-B 27 -positive AS. However, the risk of serious infections was noted, so ophthalmologists should consider the possibility that prolonged use of biologic agents may result in systemic side effects. All rights reserved. Observer variation in grading sacroiliac radiographs in HLA-B 27 positive individuals.
This study attempts to reconcile the apparent differences in the reported frequency of ankylosing spondylitis and radiological sacroilitis in HLA-B 27 positive individuals. Pelvic radiographs from Busselton subjects were mixed with 81 other films selected to illustrate the possible range of sacroiliac changes and were graded by observers who were involved in 2 of the conflicting studies and by a 3rd independent observer.
Concordance was high for advanced bilateral disease but not for unilateral and milder changes. Variation between observers and the interpretation of sacroiliac radiographs is sufficiently large to account for much of the disagreement between frequency estimates. A year-old Indian boy presented with acute and severe vision loss in his right eye. He was being treated for scalp alopecia areata and rashes behind the ears and above the brow.
The eye examination revealed unilateral hemorrhagic retinal vasculitis. The lab work was normal except for a positive HLA-B 27 result. The retinal vasculitis improved with treatment, but visual acuity only mildly improved.
The alopecia areata also improved with systemic immunosuppression. The patient complained of deterioration of visual acuity of the right eye during the last 10 days.
At presentation visual acuity of the right eye was 0. The left eye was normal with a visual acuity of 0. Eight weeks after intravitreal injection of triamcinolone acetonide, visual acuity improved to 0. Six months later no recurrence was observed. Intravitreal use of triamcinolone acetonide can reduce macular edema and restore visual acuity. The colossal number of polymorphisms in HLA molecules makes it essential to characterize the antigen-presenting properties common to large HLA families or supertypes.
The motif was independently confirmed using four unrelated peptides. This experimental approach, which could be easily transferred to other HLA class I families and supertypes, has implications for the validation of new bioinformatics tools in the functional clustering of HLA molecules, for the identification of antiviral cytotoxic T lymphocyte responses, and for future vaccine development.
The development of an animal model bearing definite antigens is important to facilitate the evaluation and modulation of specific allo-antigen responses after transplantation. Furthermore, we also demonstrated that EPA was effective in the treatment of rat cardiac allograft rejection and may allow the development of.
Magnacca, A. Non-conventional forms of HLA-B 27 are expressed in spondyloarthritis joints and gut tissue. Human leukocyte antigen HLA -B 27 B27 is the strongest genetic factor associated with development of Ankylosing Spondylitis and other spondyloarthropathies SpA , yet the role it plays in disease pathogenesis remains unclear. We investigated the expression of potentially pathogenic.
Nonetheless, there is substantial evidence that other MHC genes appear to be associated with the disease, although it has not yet been established whether these associations are driven by direct associations or by linkage disequilibrium LD mechanisms.
Allelic and haplotypic distributions were estimated by maximum likelihood method using Arlequin v3. No associations were found between non- HLA-B 27 loci and AS susceptibility, but several associations were observed for phenotypic features of the disease. Nascent HLA-class I molecules are stabilized by proteasome-derived peptides in the ER and the new complexes proceed to the cell surface through the post-ER vesicles.
Full Text Available Spondyloarthritis SpA does not display the typical features of auto-immune disease. In vitro, splenocytes were stimulated with heat-inactivated Mycobacterium tuberculosis and cytokine expression and production was measured. After validation of the model, we tested whether prophylactic and therapeutic TNF targeting affected spondylitis and arthritis.
In vitro stimulation with heat-inactivated M. In vivo immunization induced an increased spondylitis and arthritis incidence and an accelerated and synchronized onset of spondylitis and arthritis in HLA-B 27 transgenic males and females. Moreover, immunization overcame the protective effect of orchiectomy. Prophylactic TNF targeting resulted in delayed spondylitis and arthritis development and reduced arthritis severity, whereas therapeutic TNF blockade did not affect spondylitis and arthritis severity.
Collectively, these data indicate that innate immune activation plays a role in the initiation of HLA-B 27 -associated disease and allowed to establish a useful in vivo model to study the cellular and molecular mechanisms of disease initiation and progression. Positive effects of fasting and vegan diet in patients with rheumatic diseases are reported in the literature. We present a year-old patient with double-sided HLA B 27 -positive sacroiliitis, which was diagnosed by magnetic resonance tomography. Since about 10 years he therefore had pain in the iliosacral region. Numerous sessions of physiotherapy, a cure treatment, and treatment with sulfasalazine and doxycycline were not effective.
The patient was dependent on the daily intake of the nonsteroidal antirheumatics meloxicam 2 x 7. We recommended a temporary vegan diet, i. After consumption of meat 6 weeks later, complaints worsened.
